+Added: Intracellular
+Added: delivery of molecules is usually accomplished by either phagocytosis, clathrin-mediated endocytosis (CME) or through membrane
+Added: LNPs are limited in that they can typically only access a cell through the CME process, followed by disruption of the
+Added: endosomal membrane within the cell to gain access to the cytoplasm.
+Added: LNPs typically are very inefficient, and patients also experience
+Added: injection site adverse events and other toxicities associated with CME delivery, thereby limiting chronic use.
+Added: LNPs also cannot
+Added: be delivered orally.
+Added: Viral vectors, including adeno-associated virus, attempts to utilize nature’s intracellular delivery
+Added: mechanisms to facilitate fusion with the cell membrane and delivery of molecules into a cell.
+Added: Unfortunately, viral vectors have
+Added: historically been associated with severe negative immune responses and, like LNPs, cannot be delivered orally.
+Added: We believe LNCs
+Added: can effectively delivery molecules through all three mechanisms, in addition to having great flexibility with the desired route
+Added: of administration.
+Added: believe that LNC’s unique ability to enter a cell through phagocytosis, membrane fusion or CME, or a combination thereof,
+Added: relates directly to the presence of a phospholipid, like phosphatidylserine.
+Added: Phosphatidylserine (PS) is present in virtually all
+Added: cells and is an integral part of the cell membrane.
+Added: PS is normally localized to the inner part of the membrane bilayer by active
+Added: cellular processes.
+Added: However, with cell “activation”, which occurs when there is infection, inflammation, injury, stimulation,
+Added: cell death or some other issue impacting a particular cell, PS moves from the inner layer to the outer layer and facilitates fusion
+Added: with our LNCs (Figure 4).
+Added: Certain cells also contain PS receptors, which actively take up LNCs due to the presence of PS (Figure
+Added: Asymmetry of the Phospholipid Membrane
+Added: Role of PS and PS Receptors in the Uptake of LNCs into Cells
+Added: phagocytosis, macrophage and other cells containing PS receptors readily engulf LNCs and their drug cargo into vesicles, or endosomes,
+Added: facilitating intracellular delivery.
+Added: LNCs can also fuse with cell membranes and deliver drug cargo directly to the cytoplasm.
+Added: LNCs have been designed to mimic enveloped viruses and can efficiently deliver drugs and/or molecules to cells without adverse
+Added: immune responses.
+Added: some molecules, the goal is simply to achieve safe and effective intracellular delivery.
+Added: This is especially relevant when delivering
+Added: sensitive genetic material and other molecules desiring cellular impact (i.e., antivirals).
+Added: For other molecules, or drugs, utilizing
+Added: activated cells as a mechanism to deliver drug to infected tissues or other areas of the body becomes critical.
+Added: in pre-clinical studies have been shown to improve existing drugs by providing 1) cell-targeted delivery;
+Added: 2) reduced blood
+Added: levels thereby reducing toxicity;
+Added: and 3) oral delivery of drugs now only available intravenously.
+Added: For example, LNCs delivered
+Added: orally work by encapsulating molecules of drugs in a solid, anhydrous, crystalline structure, protecting them as they pass through
+Added: the GI tract where they cross the mucous membrane.
+Added: Once the LNCs have crossed the mucosal barrier of the GI tract into the lymphatic
+Added: system, they are picked up by activated cells including cells of the mononuclear phagocytic system, such as macrophages and dendritic
+Added: Activated macrophages, with drug-loaded LNCs inside, follow natural signal molecule paths and migrate to the site of infection
+Added: or to the target organ and deliver their payload.
+Added: applications of our proprietary delivery technology have been initially focused on the delivery of several potent and highly efficacious
+Added: anti-fungal and anti-bacterial agents, which are currently still associated with serious side effects, including irreversible
+Added: toxic effects on kidney and hearing function.
+Added: We believe our technology has the potential for targeted delivery of these agents,
+Added: which positions us to be at the forefront of dealing with these very serious problems.
+Added: We have now also expanded our research
+Added: and development efforts for our LNC platform delivery technology to focus on the delivery of a wide range of therapeutic treatments,
+Added: in particular those in the oligonucleotide class of agents (antisense oligonucleotides, mRNA, and CRISPR-Cas9).We continue to
+Added: push forward our business development efforts to further expand our collaborations across pharma and biotech companies who have
+Added: innovative therapies with delivery challenges, which may be addressed with our LNC platform delivery technology.
+Added: A key innovation of our LNC platform delivery technology is our ability to package medication inside lipid-crystal
+Added: particles without leaking.
+Added: Because of their crystalline nature, these particles are truly solid and hold on tightly to their medication
+Added: This is where the LNC platform delivery technology differs markedly from other lipid-based delivery technology, such
+Added: as liposomal delivery.
+Added: Liposomes are liquid delivery systems which typically leak some of their drug content into the circulatory
+Added: system, thus still exposing vulnerable organs and tissues to potential toxic effects.
+Added: Keeping potentially organ-toxic medications
+Added: inside the lipid-crystal particles significantly differentiates our LNC platform delivery technology from other drug-delivery
+Added: The size of our individual LNCs is typically in the range of 50-500 nm.
+Added: This is very small and by comparison
+Added: close to the size of a large virus or a small bacterium.
+Added: Our body produces many activated cell-types that are predisposed to interact
+Added: with our LNCs.
+Added: These activated cell types, including bone marrow-derived hematopoietic cells such as macrophages, infected cells,
+Added: injured cells, tumor cells and epithelial cells are all prone to engulf or fuse with our phosphatidylserine-based LNCs.
+Added: of the size of our LNCs and their PS surface structure (the cell membranes of bacteria are also made up from PS), activated cells
+Added: tend to take up these LNCs very efficiently and without any adverse immune response.
+Added: Many drugs that are currently on the market are only effective in treating diseases when administered intravenously.
+Added: For example, many anti-infective drugs must be administered intravenously in order to be effective.
+Added: IV administration presents
+Added: several challenges to care, such as risk of infection, patient discomfort from injections, and higher cost of care than anti-infective
+Added: drugs that can be taken orally (IV delivery must be performed by a doctor or nurse, often within a very expensive hospital setting).
+Added: Although several technologies have been used to attempt to convert IV drugs to orally delivered medications, success has been
+Added: limited due to the difficulty in achieving adequate bioavailability (i.e., the amount of drug that is absorbed into the body)
+Added: with an oral formulation.
+Added: We believe that the unique LNC structure in our platform technology protects the drug from degradation
+Added: when it passes through the GI tract and that its lipid surface features facilitate the particle being absorbed into the blood
+Added: The potential application of our LNC platform delivery technology for the delivery of injectable medications offers significant
+Added: clinical and commercial value with successfully demonstrated safety and efficacy in human clinical trials.
+Added: LNC platform technology changes the delivery of medicines in a unique manner and alters the bio-distribution of these medications
+Added: by targeting tissues and organs that are affected by infection and inflammation.
+Added: In addition to IV-only anti-infectives such as
+Added: amphotericin B and amikacin, we have orally delivered in animal studies the influenza vaccine, siRNA, NSAIDs, other anti-infectives
+Added: such as atovaquone, and many other compounds across multiple therapeutic areas, demonstrating the potential broad application
+Added: of our technology.
+Added: We have observed rapid local accumulation in infected tissues, which appear to be the result of transport of
+Added: our drug-loaded LNCs by and to activated cells.
+Added: LNC Clinical Stage Assets
+Added: have leveraged our platform LNC delivery technology to develop two clinical-stage products that we believe have the potential
+Added: to become best-in-class drugs in their respective therapeutic classes.
+Added: Our lead LNC platform delivery technology product candidate,
+Added: MAT2203, is an orally-administered LNC formulation of a broad spectrum anti-fungal drug called amphotericin B.
+Added: We believe there
+Added: are opportunities for a potentially rapid approval of MAT2203 for the treatment of certain invasive fungal infections in areas
+Added: of high unmet medical need.
+Added: In partnership with the National Institutes of Health (NIH), we have conducted numerous preclinical
+Added: studies of MAT2203 for the treatment of cryptococcal meningitis (CM), a deadly fungal infection that affects the brain, typically
+Added: in immunocompromised individuals.
+Added: In such studies, we observed the potential for MAT2203, utilizing our LNC platform delivery
+Added: technology, to (a) cross the blood-brain barrier, (b) treat this infection and (c) eliminate the toxicity normally associated
+Added: with intravenous delivery of amphotericin B.
+Added: upon the preclinical data generated by the NIH, the NIH has financially supported a grant application from the University of Minnesota
+Added: to conduct the EnACT study in Uganda.
+Added: This study was initiated in October 2019 and is exploring the use of MAT2203 for both induction
+Added: and maintenance therapy in the treatment of CM, which is one of the most frequent and opportunistic infections in HIV patients.
+Added: Given the high morbidity and mortality associated with CM in HIV patients, the clinical unmet need is globally very high with
+Added: the global burden estimated at 1 million cases annually.
+Added: We plan to leverage a 505(b)(2) regulatory pathway for MAT2203, in part
+Added: relying upon FDA’s findings of the efficacy of IV amphotericin B.
+Added: This strategy was discussed with the FDA in June 2019,
+Added: where we outlined our development plans for MAT2203 in CM and received FDA approval to proceed with the EnACT study which is currently
+Added: enrolling subjects in Cohort 2 of the trial.
+Added: We have received four qualified infectious disease (QIDP) designations as well as
+Added: an orphan designation for the treatment of cryptococcosis, which, if approved, would result in twelve years of regulatory exclusivity
+Added: We plan to seek accelerated approval for this indication following the availability of the results of the ongoing
+Added: We believe that this study may have the potential to become a potential pivotal study to support approval of MAT2203
+Added: for the treatment of CM during both induction and maintenance phases of treatment.
+Added: second clinical stage LNC-based product candidate is MAT2501, an orally administered formulation of the broad-spectrum aminoglycoside
+Added: antibiotic amikacin, which may be used to treat different types of multidrug-resistant bacteria, including non-tuberculous mycobacterium
+Added: infections (NTM), as well as various multidrug-resistant gram negative and intracellular bacterial infections.
+Added: In May 2017, we
+Added: completed and announced topline results from a Phase 1 single escalating dose clinical trial of MAT2501 in healthy volunteers
+Added: in which no serious adverse events were reported and where oral administration of MAT2501 at all tested doses yielded blood levels
+Added: that were well below the safety levels recommended for injected amikacin, supporting further development of MAT2501 for the treatment
+Added: of NTM infections.
+Added: Following reformulation work, in 2019, we received a grant from the Cystic Fibrosis Foundation (CFF) to complete
+Added: preclinical studies with Colorado State University which further demonstrated the potential for MAT2501 in treating cystic fibrosis-associated
+Added: NTM lung infections.
+Added: In November 2020, we received an additional grant from the CFF in the amount of 3.75 million USD to support
+Added: the continued development of MAT2501 through a comprehensive preclinical tox program and a single ascending dose (SAD) study in
+Added: healthy volunteers with our new and improved formulation of MAT2501.
+Added: This most recent grant was based upon the positive preclinical
+Added: proof of concept data generated by Dr.
+Added: Diane Ordway at Colorado State University in a rigorous mouse model of NTM infection in
+Added: mice with underlying CF disease.
+Added: lead anti-fungal product candidate, MAT2203, is an application of our LNC platform delivery technology to a broad spectrum and
+Added: potent anti-fungal drug called amphotericin B.
+Added: Traditionally, amphotericin B is an IV-administered drug used as a last resort
+Added: for treatment of systemic fungal infections resistant to triazoles and echinocandins, including resistant candidiasis, cryptococcal
+Added: meningoencephalitis, and aspergillosis.
+Added: To date, there have been little to no reports of clinically observed drug-resistance to
+Added: amphotericin B, further bolstering the use of this compound as the most likely last resort treatment for fungal infections in
+Added: the foreseeable future.
+Added: However, the use of amphotericin B is relatively limited because it is currently only available as an
+Added: IV-administered product and has documented history of severe toxicity (most notably nephrotoxicity).
+Added: By utilizing our LNC platform
+Added: delivery technology to nano-encapsulate amphotericin B, there is now an opportunity for the drug to be administered orally with
+Added: targeted delivery to infected cells, which we believe may have fewer side effects than the currently available IV-formulations
+Added: of amphotericin B.
+Added: Our LNC delivery of amphotericin B changes the bio-distribution, resulting in a higher level of the drug at
+Added: the site of infection and a lower level of free circulating amphotericin B.
+Added: By reducing the amount of circulating drug, our LNC
+Added: platform delivery technology may reduce overall toxicity.
+Added: Importantly, drug concentrations will be high only in target tissues
+Added: due to the migratory nature of activated cells to inflammatory regions.
+Added: Based upon our studies to date, we believe MAT2203 has
+Added: the potential to offer improved safety and reduced toxicity and, as a result, we believe MAT2203 will be able to offer a categorically
+Added: different and improved formulation that delivers orally administered amphotericin B, directly to the target cell at the site of
+Added: In collaboration with the NIH, in multiple studies, we have demonstrated in CM mouse models that our LNC-delivered
+Added: amphotericin B, following oral administration, can successfully cross the blood brain barrier to the site of infection in mice.
+Added: This demonstration provides important data indicating that our LNC platform delivery technology could become an important delivery
+Added: solution for a variety of CNS-based disorders and diseases.
+Added: believe that MAT2203 has the potential to become a best-in-class induction, consolidation, and maintenance therapy for the treatment
+Added: of CM in HIV patients by offering the following key benefits:
+Added: to treat resistant pathogens .
+Added: We believe that MAT2203 has the potential to prevent and treat fungal infections caused
+Added: by drug-resistant fungi, including those resistant to existing azoles and echinocandins, due to amphotericin B’s fungicidal
+Added: (i.e., killing the fungi) nature and potency against resistant strains and the potential for our LNC platform delivery technology
+Added: to provide higher drug exposure early in the course of therapy.
+Added: an all-oral therapy.
+Added: CM has become the most common cause of adult meningitis in many parts of Africa, where cryptococcosis
+Added: now rivals tuberculosis in all-cause mortality.
+Added: While long-term survival has improved with widespread use of antiretroviral
+Added: therapy in high-income countries, early mortality remains high.
+Added: Early mortality rates are often ~ 70% in routine practice
+Added: where access to diagnostics or medications is limited or unavailable, intracranial pressure is uncontrolled, or in settings
+Added: where other barriers to the management of CM exist.
+Added: IV administration of amphotericin B deoxycholate is not often possible
+Added: in resource-limited settings, even when it is available.
+Added: and less costly hospital stays and lower outpatient costs .
+Added: By providing physicians and patients with access to an orally
+Added: available, broad spectrum fungicidal agent in MAT2203, there is the potential to reduce hospital costs, which account for
+Added: over 70% of the overall treatment cost of invasive fungal infections.
+Added: FDA has granted MAT2203 designations for Qualified Infectious Disease Product, or QIDP, and Fast Track for the treatment of invasive
+Added: candidiasis and aspergillosis, for the prevention of invasive fungal infections in patients on immunosuppressive therapy, and
+Added: the treatment of cryptococcosis.
+Added: We recently also received Orphan Drug Designation for MAT2203 for the treatment of cryptococcosis
+Added: and associated CM.
+Added: The FDA may designate a product candidate as an orphan drug if it is intended to treat a rare disease or condition,
+Added: which is generally defined as having a patient population of fewer than 200,000 individuals in the United States, or a patient
+Added: population greater than 200,000 in the United States where there is no reasonable expectation that the cost of developing the
+Added: drug will be recovered from sales in the United States.
+Added: The orphan drug designation provides eligibility for orphan drug exclusivity
+Added: in the United States upon FDA approval if a product that has orphan drug designation subsequently receives the first FDA approval
+Added: for a particular active ingredient for the disease for which it has such designation.
+Added: For a product that obtains orphan drug designation
+Added: based on a plausible hypothesis that it is clinically superior to the same drug that is already approved for the same indication,
+Added: in order to obtain orphan drug exclusivity upon approval, clinical superiority of such product to this same drug that is already
+Added: approved for the same orphan indication must be demonstrated.
+Added: Orphan drug exclusivity means that the FDA may not approve any other
+Added: applications, including an NDA, to market the same drug for the same indication for seven years, except in limited circumstances
+Added: such as if the FDA finds that the holder of the orphan drug exclusivity has not shown that it can assure the availability of sufficient
+Added: quantities of the orphan drug to meet the needs of patients with the disease or condition for which the drug was designated.
+Added: the FDA can subsequently approve a drug with the same active moiety for the same condition during the exclusivity period if the
+Added: FDA concludes that the later drug is clinically superior, meaning the later drug is safer, more effective or makes a major contribution
+Added: to patient care.
+Added: Orphan drug designation also entitles a party to financial incentives such as opportunities for grant funding
+Added: towards clinical trial costs, a waiver from payment of user fees, an exemption from performing clinical studies in pediatric patients
+Added: unless the FDA requires otherwise by regulation, and tax credits for the cost of the clinical research.
+Added: The QIDP designation,
+Added: provided under the Generating Antibiotic Incentives Now Act, or the GAIN Act, offers certain incentives for the development of
+Added: new antibacterial or antifungal drugs, including eligibility for Fast Track designation, priority review and, if approved by the
+Added: FDA, eligibility for an additional five years of marketing exclusivity.
+Added: Fast Track designation enables more frequent interactions
+Added: with FDA to expedite drug development and review.
+Added: Fast Track designation does not change the standards for approval, and we can
+Added: provide no assurances that we can maintain Fast Track designation for MAT2203 or that such designation will result in faster regulatory
+Added: The seven-year period of marketing exclusivity provided through orphan designation, if granted, combined with an additional
+Added: five years of marketing exclusivity provided by the QIDP designation positions MAT2203 with a potential for a total of 12 years
+Added: of marketing exclusivity to be granted at the time of FDA approval.
+Added: - Product Profile
+Added: is an orally-administered, LNC formulation of amphotericin B (a broad-spectrum fungicidal agent).
+Added: Little to no clinical resistance
+Added: has been reported to date with amphotericin B as compared to the rapidly emerging drug resistance seen with other antifungal therapies.
+Added: Currently, IV administered amphotericin B is the only broad spectrum fungicidal;
+Added: however, it has significant treatment-limiting
+Added: side effects, most notably nephrotoxicity.
+Added: We believe that the ability to provide amphotericin B orally using our proprietary
+Added: and novel oral formulation comprising our LNC platform delivery technology, may offer a new and promising alternative for patients
+Added: data from animal toxicity studies for MAT2203 indicate a side-effect advantage over other amphotericin B formulations, which we
+Added: believe is based on two phenomena:
+Added: lipid- nanocrystal is a solid particle that does not significantly “leak” its drug content while circulating.
+Added: The particle releases its medication pay-load only when inside the target cells, and thus appears that the use of MAT2203
+Added: does not result in off-organ toxicities normally seen in the kidneys when using current formulations of amphotericin B.
+Added: of this targeted approach, we have been able to increase the therapeutic window on a
+Added: mg/kg basis as compared to IV amphotericin B formulations.
+Added: We have observed equivalent
+Added: efficacy at lower doses as well as been able to use oral doses of up to 10x the highest
+Added: tolerable IV dose in animal model studies.
+Added: History of MAT2203 and Initial Target Indication
+Added: has been studied extensively in animal model studies of various fungal infections including invasive candidiasis, aspergillosis,
+Added: a clinical Phase 1 single-dose, double-blind, dose-escalating, pharmacokinetic study of 48 healthy volunteers, oral MAT2203 was
+Added: observed to be well-tolerated with no serious adverse events reported, and without any observed nephrotoxicity.
+Added: The most commonly
+Added: reported adverse events (AEs) were nausea and abdominal pain.
+Added: None of the AEs were related to abnormal laboratory evaluations.
+Added: All treatment emergent adverse events (TEAEs) were mild except 1 instance of “upper respiratory tract infection” which
+Added: was moderate in a subject following 800 mg MAT2203.
+Added: No AEs led to withdrawal.
+Added: There were no serious AEs.
+Added: There was one pregnancy
+Added: (subsequently determined that the conception date was 1 to 2 days prior to dosing) resulting in elective termination from the
+Added: More recently, in our Phase 2 trial of MAT2203 conducted by the NIH, four out of four enrolled patients suffering from
+Added: chronic refractory mucocutaneous candidiasis met their primary efficacy endpoint.
+Added: One patient continues on treatment with no evidence
+Added: of kidney or other toxicity frequently associated with the use of amphotericin B.
+Added: October 2020, results from Phase 1 of the EnACT Study were published in the Journal of Antimicrobial Agents and Chemotherapy (C.
+Added: Skipper, et.al) which was a Phase 1 ascending-dose trial of MAT2203 administered at 1.0 g, 1.5 g, or 2.0 g per day in 4 to 6 divided
+Added: doses among HIV-positive survivors of cryptococcosis (n=9 per cohort).
+Added: We assessed the tolerability of MAT2203, and the AEs associated
+Added: with MAT2203 treatment over three days.
+Added: The second part of the Phase 1 trial assessed the tolerability of 1.5 grams/day (the 100%
+Added: tolerated dose) administered for seven days.
+Added: In the singe-ascending dose part of the study, all subjects in the 1.5 g treatment
+Added: group received their full dose without vomiting (100% tolerability).
+Added: The cohort receiving 1 g had 4 transient clinical AEs in
+Added: The cohort receiving 1.5 g had 7 clinical AEs in 1 subject.
+Added: The cohort receiving 2 g had 20 clinical AEs in 5 subjects.
+Added: From a qualitative survey, 26 of 27 (96%) preferred their experience with MAT2203 over their prior experience with IV amphotericin
+Added: second, multiple dose cohort received 1.5 g/day for 1 week, which was the 100% tolerated dose, with 98.4% of doses taken.
+Added: 5 clinical AEs occurred in this cohort of subjects without any observed kidney toxicity.
+Added: Oral MAT2203 was well-tolerated when
+Added: given in 4 to 6 divided daily doses without the toxicities commonly seen with IV AMB.
+Added: on the findings from Phase 1 of the EnACT Trial, 2.0 g was selected as the target dose for the induction phase of treatment and
+Added: 1.5 g for the maintenance dose for Stage 1 of the study.
+Added: Phase 2 of the trial is evaluating the safety, tolerability, and efficacy
+Added: of MAT2203 in approximately 100 HIV-infected patients with cryptococcal meningitis.
+Added: Participants enrolled in the experimental
+Added: arm of each cohort received oral MAT2203 and flucytosine (5FC) in four separate stages and duration for induction therapy, using
+Added: the maximum tolerated dose of MAT2203 that was identified in the preceding Phase 1 Trial.
+Added: The experimental arm receives MAT2203
+Added: through induction and maintenance therapy for a total of approximately 6 weeks.
+Added: Participants randomized to the control arm in
+Added: each cohort receive the 2018 WHO recommended standard of care, which is IV AMB and 5FC, followed by fluconazole.
+Added: first cohort of EnACT has been completed.
+Added: A key objective of Cohort 1 was to assess the safety and tolerability of oral MAT2203
+Added: while assessing efficacy.
+Added: Ten participants were randomized to the experimental arm and 4 to the control arm.
+Added: The study treatments
+Added: received are summarized in the following figure:
+Added: in the experimental arm received induction therapy with IV AMB (1 mg/kg/day) for the first five days, oral MAT2203 (2 g/day in
+Added: divided doses) from Day 5-14, and oral 5FC (100 mg/kg/day) for the first 14 days of treatment.
+Added: Induction therapy was followed
+Added: by consolidation or maintenance therapy with oral MAT2203 (1.5 g/day) from Day 15 to Week 6, oral fluconazole (800 mg/day) from
+Added: Day 15 to week 10, followed by oral fluconazole (200 mg/day).
+Added: and safety data from Cohort 1 of the study were reviewed by an Independent Data Safety Monitoring Board (DSMB) which voted unanimously
+Added: to progress into the second cohort of patients in the EnACT Study.
+Added: in the second cohort of patients has begun and Cohort 2.
+Added: 2 (or Stage 2) of the EnACT Study is designed to assess the potential to treat CM infections with oral MAT2203 as a step-down
+Added: treatment during the induction phase of treatment immediately following only 2 days of IV amphotericin treatment, with continued
+Added: treatment with MAT2203 for up to 6 weeks during early maintenance treatment.
+Added: We believe that the clinical benefit of step-down
+Added: treatment from IV amphotericin to oral MAT2203 will provide compelling clinical evidence of efficacy in treating this deadly infection
+Added: with our oral agent.
+Added: We believe that this Cohort of patients will also provide key data to support the further advancement of
+Added: the EnACT Study to ultimately test the potential to treat CM infections with an all-oral amphotericin dosing regimen in subsequent
+Added: cohorts (Stages 3 and 4 below).
+Added: read-out for Cohort 2 is expected in the second half of 2021.
+Added: Market Opportunity
+Added: overall global antifungal market accounted for approximately $11.9 billion in 2018 and is expected to reach approximately $13.9
+Added: billion by 2026.
+Added: In 2018, the global invasive fungal infection market was valued at more than $6 billion.
+Added: This includes therapies
+Added: used as active treatment or prophylaxis (preventative) in the inpatient and outpatient setting, therapies used for the treatment
+Added: of hospitalized patients and therapies used for the treatment of patients who are being discharged from the hospital.
+Added: that, each year, there are over 1.5 million cases of invasive fungal infections caused by various species of Candida, Aspergillus
+Added: and Cryptococcus, the three most common invasive fungal pathogens, globally.
+Added: The estimated incidence in the U.S.
+Added: these conditions is approximately 46,000 for invasive candidiasis, 15,000 for invasive aspergillosis, and 3,700 for CM.
+Added: aspergillosis-associated hospitalizations in the U.S.
+Added: alone came at an estimated treatment cost of more than $1 billion.
+Added: progression of disease and high mortality rates (20% - 50%) associated with documented invasive fungal infections often result
+Added: in antifungal therapy being administered in suspected (unconfirmed) cases or as a preventative measure in patients at high risk.
+Added: Also, the increasingly widespread use of immune suppressive drugs as cancer chemotherapy or for organ transplantation or treatment
+Added: of autoimmune disease has resulted in an increasing population of patients at risk for invasive fungal infections.
+Added: the limited number of systemic antifungal drug classes, consisting of azoles, echinocandins and polyenes, and their extensive
+Added: use, has led to increased numbers of infections with drug-resistant strains.
+Added: The Centers for Disease Control and Prevention (CDC)
+Added: has listed fluconazole-resistant Candida as a serious threat requiring prompt and sustained action and has also identified
+Added: a rise in echinocandin resistance, especially among Candida glabrata.
+Added: In June 2016, the CDC issued an extraordinary alert
+Added: for healthcare facilities and providers to be on the lookout for patients with Candida auris , a multidrug resistant strain
+Added: with high mortality (approximately 60%).
+Added: Almost half of C.
+Added: auris isolates are multidrug resistant to two or more antifungal
+Added: classes (large majority resistant to fluconazole, 40% resistant to echinocandins).
+Added: We believe this underscores the urgent need
+Added: for new agents with demonstrated activity against resistant strains and that can be administered with significantly less toxicity
+Added: and the potential to discharge patients earlier to reduce hospital stays and associated costs.
+Added: options for the treatment of fungal infections are limited by a lack of innovative therapies.
+Added: Several factors have contributed
+Added: to the low rate of antifungal drug development, including a previously challenging regulatory environment that necessitated large
+Added: and costly clinical trials.
+Added: As a result of this regulatory environment and other factors, the number of antifungals in development
+Added: has decreased, while anti-microbial resistance has increased.
+Added: is an oral, LNC formulation of the broad-spectrum aminoglycoside antibiotic agent amikacin, which utilizes our proprietary LNC
+Added: platform to achieve oral bioavailability, limit toxicity, and enable targeted delivery to sites of infection.
+Added: Currently, amikacin
+Added: can only be delivered parenterally or through inhalation and is used to treat a variety of chronic and acute bacterial infections,
+Added: including both NTM infections and various multi-drug resistant gram-negative bacterial infections.
+Added: IV and inhaled amikacin, however,
+Added: are associated with major side-effects including nephrotoxicity and ototoxicity (permanent loss of hearing) with long-term use.
+Added: We believe that MAT2501’s ability to orally deliver high levels of amikacin directly to the lung and without use-limiting
+Added: toxicity, distinguishes it from all available therapies and could provide an important solution for patients and physicians.
+Added: are currently developing MAT2501 for the treatment of NTM lung disease, including infections in patients with CF.
+Added: been designated as a Qualified Infectious Disease Product (QIDP) and as an Orphan Drug for the treatment of NTM by the US FDA.
+Added: lung disease is a chronic, debilitating condition arising from an NTM infection in the lungs and is associated with significant
+Added: patient morbidity and mortality.
+Added: The signs and symptoms of NTM lung disease often overlap with the underlying lung conditions
+Added: that increase the risk for NTM, like cystic fibrosis, bronchiectasis, COPD, and asthma.
+Added: The most common pathogens for NTM infections
+Added: in the United States are Mycobacterium avium complex (MAC), which accounts for more than 80% of all NTM infections in the
+Added: Patients with NTM lung infections frequently require lengthy hospital stays and prolonged courses of antibiotics to manage
+Added: their disease.
+Added: The prevalence of human disease attributable to NTM has increased over the past two decades and is now growing
+Added: at more than 8% per year and is even more prevalent than tuberculosis in the US.
+Added: In 2018, it was estimated that between 75,000
+Added: and 100,000 patients were diagnosed with NTM lung disease in the US alone.
+Added: Non-tuberculous
+Added: mycobacterium (NTM) infections are extremely difficult to treat, especially so in patients with cystic fibrosis (Eikani, et.
+Added: The infecting organisms are frequently resistant to most antibiotics, and current treatment regimens require combination
+Added: therapies with highly toxic drugs for long periods of time, further complicated by challenges in delivering therapeutic levels
+Added: of these toxic drugs across plasma membranes of infected cells.
+Added: challenges are amplified in CF patients, with the thick buildup of pulmonary secretions that further impair treatment of infecting
+Added: Pulmonary infections represent the most frequent type of infection in CF patients, and are responsible for more than
+Added: 90% of deaths in the CF population (Rowe SM, et.al .
+Added: Mycobacterium avium complex (MAC) and Mycobacterium abscessus
+Added: complex (MABSC) are non-tuberculous mycobacterial species that have emerged in recent years as important opportunistic pathogens
+Added: frequently responsible for pulmonary infections in CF patients (Brode SK, et.
+Added: Infections due to MAC and MABSC
+Added: are difficult to treat and to eradicate since these organisms are naturally resistant to most antibiotics.
+Added: The recommended treatment
+Added: for MAC and MABSC pulmonary infections includes a combination of a macrolide (clarithromycin or azithromycin), an aminoglycoside
+Added: (amikacin), and an antimycobacterial antibiotic for MAC (rifampin and ethambutol), while MABSC many times requires intravenous
+Added: β-lactam (cefoxitin or imipenem) at least for 12 months (Floto RA, et.
+Added: The cure rate is only 25-40% in
+Added: the case of macrolide resistance, which is present in 40 to 60% of the isolates (Roux AL et.
+Added: Moreover, unsuccessful
+Added: eradication of MAC and MABSC is considered as a contraindication for lung transplantation by several CF centers, and significantly
+Added: restrict therapeutic options in severely ill patients.
+Added: Therefore, more specific and active and less toxic antimicrobials are urgently
+Added: Previous and Ongoing Studies
+Added: most recent completed pre-clinical animal model studies testing the preclinical efficacy of MAT2501 were funded by a grant from
+Added: the Cystic Fibrosis Foundation (CFF).
+Added: An additional grant was awarded from the CFF in February of 2020 to support the screening
+Added: of new optimized formulations of MAT2501 in collaboration with Diane Ordway, Ph.D.
+Added: of Colorado State University (CSU).
+Added: Most recently,
+Added: a CFF grant of $3.75 million was awarded to us in November 2020 to support the early development of MAT2501 through the end of
+Added: The recent grant from the CF Foundation was based upon the data generated by CSU and summarized below.
+Added: and Clinical Studies
+Added: Investigational New Drug (IND) application for MAT2501, was opened in January 2016 to investigate MAT2501 in the proposed indication
+Added: for treatment of non-tuberculosis mycobacterial (NTM) infections.
+Added: have conducted numerous repeat-dose non-clinical studies including a 28-Day rat study and 3 nonclinical studies in large animals
+Added: (2 with dog and 1 with dog and minipig).
+Added: Data generated to date suggest no toxicity signals related to those observed with IV
+Added: have conducted to date a first-in-human Phase I study entitled:
+Added: A Phase 1, Double-Blind, Placebo- Controlled, Single, Ascending
+Added: Dose Study to Evaluate the Safety, Tolerability, and Pharmacokinetics of Encochleated Amikacin (CAMK/MAT2501) Following Oral Administration
+Added: in Healthy Adult Subjects.
+Added: Safety was monitored through physical examinations, vital signs, 12-lead ECGs, audiometry tests, pupillary
+Added: reflex tests and clinical lab tests.
+Added: No serious adverse events were reported in the study.
+Added: Adverse events were tolerable and not
+Added: There were no adverse events related to audiology assessments or ophthalmic pupillary reflex testing.
+Added: A total of 36
+Added: subjects were entered into this study in three (3) cohorts (dosing of 12 subjects (9 active drug subjects and 3 placebo subjects
+Added: in each cohort).
+Added: Doses tested were 200mg under fasted conditions, 400mg under fasted and fed conditions and 800mg under fed conditions.
+Added: One subject in high dose cohort (800 mg dose, fed condition) was discontinued early during the study as a result of gastrointestinal
+Added: issues possibly related to study drug.
+Added: The study was completed in March of 2017 and the final clinical study report was submitted
+Added: to the IND in February of 2018.

+Added: counts showed that the oral administration of MAT2501 100 mg/kg resulted in CFU lung, spleen and liver counts that were lower
+Added: than those in mice receiving parenteral amikacin alone.
+Added: counts showed that the oral administration BID of MAT2501 resulted in CFU lung, spleen and liver counts that were significantly
+Added: lower than those in mice receiving clarithromycin alone.
+Added: lung pathology in cystic fibrosis mice infected with each of the NTM isolates evaluated in this study showed that lesions
+Added: were more numerous and larger in infected mice that were treated with clarithromycin or parenteral amikacin compared to the
+Added: smaller lesions after oral administration of MAT2501.
+Added: Treatment Guidelines and Limitations of Current Treatments
+Added: American Thoracic Society (ATS), European Respiratory Society (ERS), European Society of Clinical Microbiology and Infectious
+Added: Diseases (ESCMIID), and Infectious Diseases Society of America (IDSA) jointly sponsored the recent development of updated NTM
+Added: Treatment Guidelines in adults (Daley C, et.al , 2020).
+Added: The guidelines recommend a standard NTM lung disease treatment with
+Added: a combination of 3-4 medications.
+Added: These include Amikacin (or streptomycin), clarithromycin (or azithromycin), rifampin (or rifabutin),
+Added: and ethambutol.
+Added: Amikacin is an important drug used for treatment of Mycobacterium abscessus pulmonary disease.
+Added: combinations of antibiotics work better together because of their unique mechanisms of action that work in concert to eliminate
+Added: the infection.
+Added: It is for this reason that sputum samples are qualified for precise identification of the species and sensitivity
+Added: testing of the microorganism.

−Removed: greater bioavailability than ethyl-ester forms under the guideline-recommended diet conditions for patients with elevated
−Removed: blood lipids.
−Removed: Our prior head-to-head PK/PD trial compared the bioavailability of MAT9001 and Vascepa (an ethyl ester formulation)
−Removed: on a low-fat diet;
−Removed: as previously noted patients taking MAT9001 had five times higher blood plasma levels of EPA in comparison
−Removed: to patients taking Vascepa.
−Removed: Additional benefits of MAT9001’s superior bioavailability may include flexible dosing (once
−Removed: or twice a day, not dependent on meals), and, if lower doses of a more bioavailable compound give comparable TG reductions
−Removed: to existing therapies, a reduced pill burden and accompanying heightened patient compliance.
−Removed: As a direct consequence of both its high bioavailability (and its ability as
−Removed: a free fatty acid formulation to generate higher blood levels of EPA than other available
−Removed: products) and the more potent TG-lowering properties of DPA, MAT9001 has been specifically
−Removed: designed to provide superior degrees of TG-lowering than other available products in
−Removed: the omega-3 class.
−Removed: This superiority has already been highlighted in our initial head-to-head
−Removed: study versus Vascepa, and we expect to validate these findings in ENHANCE-IT, an upcoming
−Removed: second head-to-head study.
−Removed: Encapsulation
−Removed: One challenge in optimizing our free fatty acid formulation of MAT9001 is the fact that free fatty acids in general
−Removed: tend to be more volatile than their ethyl ester counterparts, and could, potentially, be associated with more risk for accompanying
−Removed: gastrointestinal (GI) symptoms.
−Removed: In order to overcome this, delayed release technology is applied to reduce exposure
−Removed: in the stomach and to ensure that MAT9001 is delivered farther along the GI tract, where it will be better tolerated
−Removed: and optimally absorbed.
−Removed: Our unique proprietary capsule technology incorporates the delayed release technology as part of the
−Removed: soft gelatin capsule and potentially creates a more stable, more reliable protection for the free fatty acid contents, and
−Removed: minimizes the risks of adverse GI symptoms.
−Removed: summary, we believe that MAT9001, with its unique composition of omega-3 free fatty acids, its increased plasma concentrations
−Removed: of EPA compared to Vascepa, its potential for once-a-day dosing and flexible/convenient administration (not requiring administration
−Removed: with food), and its already-documented high bioavailability and superior potency in reducing triglycerides in studies to date,
−Removed: is well-positioned to address significant unmet medical needs and become a standard of care in the treatment of patients with
−Removed: Furthermore, given its potential unique synergism with statins (which tend to elevate PCSK9) we believe that MAT9001
−Removed: has significant potential as a future first-line addition to statins in the treatment of mixed dyslipidemia.
−Removed: Platform Delivery Technology
−Removed: have leveraged our platform LNC platform delivery technology to develop two clinical-stage products that we believe have the potential
−Removed: to become best-in-class drugs in their respective therapeutic classes.
−Removed: Our lead LNC platform delivery technology product candidate,
−Removed: MAT2203, is an orally-administered LNC formulation of a broad spectrum anti-fungal drug called amphotericin B.
−Removed: We believe there
−Removed: are opportunities for a potentially rapid approval of MAT2203 for the treatment of certain invasive fungal infections in areas
−Removed: of high unmet medical need which can be substantially supported by non-dilutive government funds.
−Removed: In partnership with the National
−Removed: Institutes of Health (NIH), we have conducted numerous preclinical studies of MAT2203 for the treatment of cryptococcal meningitis
−Removed: (CM), a deadly fungal infection that affects the brain, typically in immunocompromised individuals.
−Removed: In such studies, we observed
−Removed: the potential for MAT2203, utilizing our LNC platform delivery technology, to (a) cross the blood-brain barrier, (b) treat this
−Removed: infection and (c) eliminate the toxicity normally associated with intravenous delivery of amphotericin B.
−Removed: upon the preclinical data generated by the NIH, the NIH has financially supported a grant application from the University of Minnesota
−Removed: to conduct the EnACT study in Uganda.
−Removed: This study was initiated in October 2019 and is exploring the use of MAT2203 for
−Removed: both induction and maintenance therapy in the treatment of CM, CM is one of the most frequent and opportunistic infections in
−Removed: HIV patients.
−Removed: Given the high morbidity associated with CM in HIV patients, the clinical unmet need is globally very high with
−Removed: the global burden estimated at 1 million cases annually.
−Removed: We plan to leverage the 505(b)(2) regulatory pathway for MAT2203, in
−Removed: part relying upon FDA’s findings of the efficacy of amphotericin B.
−Removed: This strategy was discussed with the FDA in June 2019
−Removed: where we outlined our development plans for MAT2203 in CM and received FDA approval to proceed with the EnACT study.
−Removed: We have received
−Removed: orphan designation for the CM indication and we plan to seek accelerated approval for this indication following the availability
−Removed: of the results of the ongoing EnACT Study.
−Removed: We believe that this study may have the potential to become a pivotal study to support
−Removed: approval of MAT2203 for the treatment of CM.
−Removed: second clinical stage LNC-based product candidate is MAT2501, an orally administered formulation of the broad-spectrum aminoglycoside
−Removed: antibiotic amikacin which may be used to treat different types of multidrug-resistant bacteria, including non-tuberculous mycobacterium
−Removed: infections (NTM), as well as various multidrug-resistant gram negative and intracellular bacterial infections.
−Removed: This formulation
−Removed: also applies our proprietary LNC platform delivery technology.
−Removed: In May 2017, we completed and announced topline results from a
−Removed: Phase 1 single escalating dose clinical trial of MAT2501 in healthy volunteers in which no serious adverse events were reported
−Removed: and where oral administration of MAT2501 at all tested doses yielded blood levels that were well below the safety levels recommended
−Removed: for injected amikacin, supporting further development of MAT2501 for the treatment of NTM infections.
−Removed: In 2019, following a grant
−Removed: from the Cystic Fibrosis Foundation (CFF), we completed preclinical studies with Colorado State University which further demonstrated
−Removed: the potential for MAT2501 in treating cystic fibrosis-associated NTM lung infections.
−Removed: Following review of this data and discussions
−Removed: with the CFF, we are in the process of applying for a grant from the CFF to fund development of MAT2501 through Phase 2.
−Removed: - Product Profile
−Removed: is an orally-administered, LNC formulation of amphotericin B (a broad-spectrum fungicidal agent).
−Removed: Little to no clinical resistance
−Removed: has been reported to date with amphotericin B as compared to the rapidly emerging drug resistance seen in other antifungal therapies.
−Removed: Currently, IV administered amphotericin B is the only broad spectrum fungicidal;
−Removed: however, it has significant treatment-limiting
−Removed: side effects, most notably nephrotoxicity.
−Removed: The ability to provide amphotericin B orally using our proprietary and novel oral formulation
−Removed: comprising our LNC platform delivery technology, may offer a new and promising alternative for patients and doctors.
−Removed: In a clinical
−Removed: Phase 1 single-dose, double-blind, dose-escalating, pharmacokinetic study of 48 healthy volunteers, oral MAT2203 was observed
−Removed: to be well tolerated with no serious adverse events reported, and without any observed nephrotoxicity.
−Removed: The most commonly reported
−Removed: AEs were nausea and abdominal pain.
−Removed: None of the AEs were related to abnormal laboratory evaluations.
−Removed: All treatment emergent adverse
−Removed: events (TEAEs) were mild except 1 instance of “upper respiratory tract infection”
−Removed: which was moderate in a subject
−Removed: following 800 mg MAT2203.
−Removed: No AEs led to withdrawal.
−Removed: There were no serious AEs.
−Removed: There was one pregnancy (subsequently determined
−Removed: that the conception date was 1 to 2 days prior to dosing) resulting in elective termination from the study.
−Removed: More recently, in
−Removed: our Phase 2 trial of MAT2203 conducted by the NIH, four out of four enrolled patients suffering from chronic refractory
−Removed: mucocutaneous candidiasis met their primary efficacy endpoint.
−Removed: Three patients continue on treatment, of which
−Removed: two have been successfully taking MAT2203 for more than three years as part of a long-term safety extension, with no evidence
−Removed: of kidney or other toxicity frequently associated with the use of amphotericin B.
−Removed: Market Opportunity
−Removed: overall global antifungal market accounted for approximately $11.9 billion in 2018 and is expected to reach approximately $13.9
−Removed: billion by 2026.
−Removed: In 2018, the global invasive fungal infection market was valued at more than $6 billion.
−Removed: This includes therapies
−Removed: used as active treatment or prophylaxis (preventative) in the inpatient and outpatient setting, therapies used for the treatment
−Removed: of hospitalized patients and therapies used for the treatment of patients who are being discharged from the hospital.
−Removed: that, each year, there are over 1.5 million cases of invasive fungal infections caused by various species of Candida, Aspergillus
−Removed: and Cryptococcus, the three most common invasive fungal pathogens, globally.
−Removed: The estimated incidence in the U.S.
−Removed: these conditions is approximately 46,000 for invasive candidiasis, 15,000 for invasive aspergillosis, and 3,700 for CM.
−Removed: aspergillosis-associated hospitalizations in the U.S.
−Removed: alone came at an estimated treatment cost of more than $1 billion.
−Removed: progression of disease and high mortality rates (20% - 50%) associated with documented invasive fungal infections often result
−Removed: in antifungal therapy being administered in suspected (unconfirmed) cases or as a preventative measure in patients at high risk.
−Removed: Also, the increasingly widespread use of immune suppressive drugs as cancer chemotherapy or for organ transplantation or treatment
−Removed: of autoimmune disease has resulted in an increasing population of patients at risk for invasive fungal infections.
−Removed: the limited number of systemic antifungal drug classes, consisting of azoles, echinocandins and polyenes, and their extensive
−Removed: use, has led to increased numbers of infections with drug-resistant strains.
−Removed: The Centers for Disease Control and Prevention (CDC)
−Removed: has listed fluconazole-resistant Candida as a serious threat requiring prompt and sustained action and has also identified
−Removed: a rise in echinocandin resistance, especially among Candida glabrata.
−Removed: In June 2016, the CDC issued an extraordinary alert
−Removed: for healthcare facilities and providers to be on the lookout for patients with Candida auris , a multidrug resistant strain
−Removed: with high mortality (approximately 60%).
−Removed: Almost half of C.
−Removed: auris isolates are multidrug resistant to two or more antifungal
−Removed: classes (large majority resistant to fluconazole, 40% resistant to echinocandins).
−Removed: We believe this underscores the urgent need
−Removed: for new agents with demonstrated activity against resistant strains and that can be administered with significantly less toxicity
−Removed: and the potential to discharge patients earlier to reduce hospital stays and associated costs.
−Removed: Physicians’
−Removed: options for the treatment of fungal infections are limited by a lack of innovative therapies.
−Removed: Several factors have contributed
−Removed: to the low rate of antifungal drug development, including a previously challenging regulatory environment that necessitated large
−Removed: and costly clinical trials.
−Removed: As a result of this regulatory environment and other factors, the number of antifungals in development
−Removed: has decreased, while anti-microbial resistance has increased.
−Removed: Solution –
−Removed: lead anti-infective product candidate, MAT2203, is an application of our LNC platform delivery technology to a broad spectrum
−Removed: anti-fungal drug called amphotericin B.
−Removed: Traditionally, amphotericin B is an IV-administered drug used as a last resort for treatment
−Removed: of systemic fungal infections resistant to triazoles and echinocandins, including resistant candidiasis, cryptococcal meningoencephalitis,
−Removed: and aspergillosis.
−Removed: To date, there have been little to no reports of clinically observed drug-resistance to amphotericin B, further
−Removed: bolstering the use of this compound as the most likely last resort treatment for fungal infections in the foreseeable future.
−Removed: However, the use of amphotericin B is relatively limited because it is currently only available as an IV-administered product
−Removed: and has documented history of severe toxicity (most notably nephrotoxicity).
−Removed: By utilizing our LNC platform delivery technology
−Removed: to nano-encapsulate amphotericin B, there is now an opportunity for the drug to be administered orally with targeted delivery
−Removed: to infected cells, which we believe may have fewer side effects than the currently available IV-formulations of amphotericin B.
−Removed: Our LNC delivery of amphotericin B changes the bio-distribution, resulting in a higher level of the drug at the site of infection
−Removed: and a lower level of circulating amphotericin B.
−Removed: By reducing the amount of circulating drug, our LNC platform delivery technology
−Removed: may reduce overall toxicity.
−Removed: Importantly, drug concentrations will be high only in tissues due to the migratory nature of macrophages
−Removed: to inflammatory regions.
−Removed: Based upon our studies to date, we believe MAT2203 has the potential to offer improved safety and reduced
−Removed: toxicity and, as a result, we believe MAT2203 will be able to offer a categorically different formulation that delivers orally
−Removed: administered amphotericin B, directly to the target cell at the site of infection.
−Removed: In collaboration with the NIH, in multiple
−Removed: studies, we have demonstrated in CM mouse models that our LNC-delivered amphotericin B, following oral administration, has the
−Removed: ability to successfully cross the blood brain barrier to the site of infection in mice.
−Removed: This demonstration provides important
−Removed: data indicating that our LNC platform delivery technology could become an important delivery solution for a variety of CNS-based
−Removed: disorders and diseases.
−Removed: believe that MAT2203 has the potential to become a best-in-class induction, consolidation, and maintenance therapy for the treatment
−Removed: of CM in HIV patients by offering the following key benefits:
−Removed: to treat resistant pathogens .
−Removed: We believe that MAT2203 has the potential to prevent and treat fungal infections caused
−Removed: by drug-resistant fungi, including those resistant to existing azoles and echinocandins, due to amphotericin B’s fungicidal
−Removed: killing the fungi) nature and potency against resistant strains and the potential for our cochleate drug delivery platform
−Removed: to provide higher drug exposure early in the course of therapy.
−Removed: an all-oral therapy.
−Removed: CM has become the most common cause of adult meningitis in many parts of Africa, where cryptococcosis
−Removed: now rivals tuberculosis in all-case mortality.
−Removed: While long-term survival has improved with widespread use of antiretroviral
−Removed: therapy in high income countries, early mortality remains high.
−Removed: Early mortality rates are often ~ 70% in routine practice
−Removed: where access to diagnostics or medications is limited or unavailable, intracranial pressure is uncontrolled, or in settings
−Removed: where other barriers to the management of CM exist.
−Removed: IV administration of amphotericin B deoxycholate is not often possible
−Removed: in resource-limited settings, even when it is available.
−Removed: and less costly hospital stays and lower outpatient costs .
−Removed: By providing physicians and patients with access to an orally
−Removed: available, broad spectrum fungicidal agent in MAT2203, there is the potential to reduce hospital costs, which account for
−Removed: over 70% of the overall treatment cost of invasive fungal infections.
−Removed: FDA has granted MAT2203 designations for Qualified Infectious Disease Product, or QIDP, and Fast Track for the treatment of invasive
−Removed: candidiasis and aspergillosis, for the prevention of invasive fungal infections in patients on immunosuppressive therapy, and
−Removed: the treatment of cryptococcosis.
−Removed: We recently also received Orphan Drug Designation for MAT2203 for the treatment of CM.
−Removed: may designate a product candidate as an orphan drug if it is intended to treat a rare disease or condition, which is generally
−Removed: defined as having a patient population of fewer than 200,000 individuals in the United States, or a patient population greater
−Removed: than 200,000 in the United States where there is no reasonable expectation that the cost of developing the drug will be recovered
−Removed: from sales in the United States.
−Removed: The orphan drug designation provides eligibility for orphan drug exclusivity in the United States
−Removed: upon FDA approval if a product that has orphan drug designation subsequently receives the first FDA approval for a particular
−Removed: active ingredient for the disease for which it has such designation.
−Removed: For a product that obtains orphan drug designation on the
−Removed: basis of a plausible hypothesis that it is clinically superior to the same drug that is already approved for the same indication,
−Removed: in order to obtain orphan drug exclusivity upon approval, clinical superiority of such product to this same drug that is already
−Removed: approved for the same orphan indication must be demonstrated.
−Removed: Orphan drug exclusivity means that the FDA may not approve any other
−Removed: applications, including an NDA, to market the same drug for the same indication for seven years, except in limited circumstances
−Removed: such as if the FDA finds that the holder of the orphan drug exclusivity has not shown that it can assure the availability of sufficient
−Removed: quantities of the orphan drug to meet the needs of patients with the disease or condition for which the drug was designated.
−Removed: the FDA can subsequently approve a drug with the same active moiety for the same condition during the exclusivity period if the
−Removed: FDA concludes that the later drug is clinically superior, meaning the later drug is safer, more effective or makes a major contribution
−Removed: to patient care.
−Removed: Orphan drug designation also entitles a party to financial incentives such as opportunities for grant funding
−Removed: towards clinical trial costs, a waiver from payment of user fees, an exemption from performing clinical studies in pediatric patients
−Removed: unless the FDA requires otherwise by regulation, and tax credits for the cost of the clinical research.
−Removed: The QIDP designation,
−Removed: provided under the Generating Antibiotic Incentives Now Act, or the GAIN Act, offers certain incentives for the development of
−Removed: new antibacterial or antifungal drugs, including eligibility for Fast Track designation, priority review and, if approved by the
−Removed: FDA, eligibility for an additional five years of marketing exclusivity.
−Removed: Fast Track designation enables more frequent interactions
−Removed: with FDA to expedite drug development and review.
−Removed: Fast Track designation does not change the standards for approval and we can
−Removed: provide no assurances that we can maintain Fast Track designation for MAT2203 or that such designation will result in faster regulatory
−Removed: The seven-year period of marketing exclusivity provided through orphan designation, if granted, combined with an additional
−Removed: five years of marketing exclusivity provided by the QIDP designation positions MAT2203 with a potential for a total of 12 years
−Removed: of marketing exclusivity to be granted at the time of FDA approval.
−Removed: Our plan is to further secure QIDP/Fast Track/Orphan Designation
−Removed: for the initial development target indication of CM.
−Removed: History of MAT2203 and Initial Target Indication
−Removed: was studied in animal model studies of various fungal infections including invasive candidiasis, aspergillosis and CM.
−Removed: data from animal studies for MAT2203 indicate a side-effect advantage over other amphotericin B formulations, which we believe
−Removed: is based on two phenomena:
−Removed: lipid-crystal nano-particle is a solid particle that does not significantly “leak”
−Removed: its drug content while circulating.
−Removed: The particle releases its medication pay-load only when inside the target cells, and thus appears that the use of MAT2203
−Removed: does not result in toxicities normally seen in the kidneys when using current formulations of amphotericin B.
−Removed: of this targeted approach, we have been able to increase the therapeutic window on a mg/kg basis as compared to IV amphotericin
−Removed: B formulations.
−Removed: We have observed equivalent efficacy at lower doses as well as been able to use oral doses of up to 10x the
−Removed: highest tolerable IV dose in animal model studies.
−Removed: NIH-Conducted
−Removed: early 2017, we reported interim data from the NIH-conducted phase 2a clinical study of orally-administered MAT2203
−Removed: for the treatment of chronic refractory mucocutaneous candidiasis.
−Removed: At that time, two out of the two patients with
−Removed: long-standing azole resistant mucocutaneous candidiasis met the primary endpoint of the Phase 2a study, achieving ≥
−Removed: response with treatment of MAT2203.
−Removed: Patient #01 achieved a 57% reduction in clinical symptoms after 8 weeks on therapy while patient
−Removed: #02 achieved an 85% reduction in such clinical symptoms after 6 weeks of treatment.
−Removed: MAT2203 was well tolerated with majority of
−Removed: adverse events observed being mild in severity and mostly unrelated to study drug.
−Removed: Importantly, for both patients renal and liver
−Removed: function parameters remained well within normal ranges during the core study as well as during the first 6-month extension of
−Removed: In July 2017, the NIH/NIAID institutional review board approved continuation of treatment of patients in the study-extension
−Removed: for an additional 6 months, for total extension of up to one year.
−Removed: January 2018, the NIH reported positive data from a third patient enrolled in this study.
−Removed: This third patient, with long-standing
−Removed: azole resistant mucocutaneous candidiasis, met the primary endpoint of the Phase 2a study in achieving ≥
−Removed: 50% clinical response
−Removed: with treatment of MAT2203.
−Removed: MAT2203 was well tolerated with any adverse events observed being mild in severity and unrelated to
−Removed: With this third positive response, the study has met its statistical hurdle for success.
−Removed: In June of 2018, the NIH
−Removed: reported that a fourth patient had enrolled in the study and had met the primary endpoint in achieving ≥
−Removed: 50% clinical response
−Removed: with treatment of MAT2203.
−Removed: All four patients had been enrolled in a long-term study extension and the initial two patients have
−Removed: now shown no signs of kidney or liver toxicity over the approximately twenty-four months of being administered MAT2203.
−Removed: patient was required to drop out of the long-term safety portion of the study due to the development of an infection that does
−Removed: not respond to amphotericin B.
−Removed: The fourth patient continues in the long-term safety extension for the study.
−Removed: The clinical response
−Removed: to MAT2203 seen in all three patients continuing on drug has been maintained and/or improved during the extension period in addition
−Removed: to patients reporting meaningful quality-of-life improvements.
−Removed: late 2017, we announced the topline data from our phase 2 study in vulvovaginal candidiasis (VVC) using MAT2203.
−Removed: In the context of our overall program for MAT2203 with the aim to develop our lead product initially for the treatment of invasive
−Removed: fungal infections, our goal was, in addition to further establishing the safety and tolerability of MAT2203, to demonstrate efficacy
−Removed: of MAT2203 through a mechanism involving systemic absorption in a non-life threatening fungal infection.
−Removed: This study concept was
−Removed: consistent with early human efficacy studies in the development of other anti- fungal therapies.
−Removed: This Phase 2 study was not designed
−Removed: or powered to support an indication for the treatment of VVC and therefore supplant fluconazole as the standard of care.
−Removed: data generated from this study included additional safety and tolerability data.
−Removed: this VVC study, the primary endpoint of safety was met, and it was demonstrated that oral delivery of encochleated amphotericin
−Removed: B is safe and well tolerated without the renal and hepatic toxicities that can be seen with administration of intravenous amphotericin
−Removed: June 2019, we met with FDA to review our development plan and study design for the EnACT study as well as our plans to leverage
−Removed: a 505(b)(2) pathway, relying, in part, upon FDA’s findings of safety and efficacy of I.V.
−Removed: amphotericin B.
−Removed: FDA provided positive feedback on our regulatory strategy and clinical development plan to conduct a next study in CM.
−Removed: The EnACT Study was initiated in October 2019 and is currently being conducted in Uganda under a US investigational new drug application
−Removed: In February 2020, we announced that the phase 1 portion of the EnACT study had been completed with no significant tolerability
−Removed: or safety issues observed.
−Removed: Data from the dose escalation phase of the study demonstrated that MAT2203 was safe and well tolerated
−Removed: across all three doses tested (1g, 1.5g and 2g).
−Removed: Also in February of 2020, the independent Data Monitoring Safety Board (DSMB)
−Removed: for the study unanimously approved the commencement of phase 2 of the EnACT trial which is designed to explore the use of MAT2203
−Removed: for both induction and maintenance therapy in HIV-patients with CM.
−Removed: efficacy portion of the study, or phase 2, has been designed as an open-label trial to evaluate the safety, tolerability, and
−Removed: microbiologic efficacy of oral MAT2203 as part of induction and consolidation treatment of HIV-infected patients with CM compared
−Removed: with standard intravenously delivered amphotericin B.
−Removed: Participants in the study will be enrolled in sequential cohorts designed
−Removed: to mitigate the risk associated with these very sick patients.
−Removed: Induction treatment in each cohort will start with IV amphotericin
−Removed: and flucytosine treatment with oral MAT2203 administered as step-down treatment for the initial two cohorts (with earlier step-down
−Removed: to MAT2203 in each subsequent cohort.) The next two cohorts will test the induction of treatment with our MAT2203 product, with
−Removed: step-down treatment to IV amphotericin.
−Removed: The final cohort of patients will have a MAT2203 induction treatment (plus flucytosine)
−Removed: without IV administered amphotericin B (all-oral treatment).
−Removed: The primary endpoint for this trial will be the rate of cerebrospinal
−Removed: fluid (CSF) cryptococcus clearance as measured by serial quantitative CSF fungal cultures.
−Removed: expect to announce progression from cohort to cohort in the EnACT trial during 2020 and 2021 without releasing individual data
−Removed: We anticipate completion of the EnACT trial in late 2021.
−Removed: addition to the clinical portion of our development program, we have also completed a 90-day preclinical toxicology study to support
−Removed: dosing with MAT2203 in the EnACT study.
−Removed: In both the preclinical work and in phase 1 of the EnACT study, there have been no signs
−Removed: Cochleate Platform Delivery Technology
−Removed: lipid-crystal nano-particles are composed of simple, naturally occurring materials:
−Removed: phosphatidylserine (PS) and calcium.
−Removed: are stable and have a unique multilayered structure consisting of a large, continuous, solid, lipid bilayer sheet rolled up in
−Removed: a spiral or as stacked sheets, with no internal aqueous space (Figure 1).
−Removed: This unique structure provides protection from degradation
−Removed: for “encochleated”
−Removed: Components within the interior of the cochleate remain intact, even though the outer
−Removed: layers of the cochleate may be exposed to harsh environmental conditions or enzymes.
−Removed: 1 Cochleate Formulation
−Removed: structure is formed when a series of solid lipid sheets engulf drug molecules, a process referred to as “encochleation.”
−Removed: Encochleation, developed by Matinas and Rutgers New Jersey Medical School, involves combining calcium and soy-derived PS, two
−Removed: naturally occurring materials classified as GRAS (generally recognized as safe), through a stirring process to envelop the active
−Removed: pharmacological ingredient.
−Removed: The result is a nano-size encochleated drug formulation (Figure 2).
−Removed: 2 Formation of Cochleate
−Removed: have been shown to improve existing drugs by providing 1) cell-targeted delivery;
−Removed: 2) reduced blood levels thereby reducing toxicity;
−Removed: and 3) oral delivery of drugs now only available intravenously.
−Removed: Cochleates work by encapsulating molecules of drugs in a solid,
−Removed: anhydrous, crystalline structure, protecting them as they pass through the GI tract where they cross the mucous membrane.
−Removed: the cochleates have crossed the mucosal barrier of the GI tract into the lymphatic system, they are picked up by particle scavenging
−Removed: cells of the mononuclear phagocytic system, such as macrophages and dendritic cells.
−Removed: Activated macrophages, with drug-cochleate
−Removed: inside, migrate to the site of infection or to the target organ and deliver amphotericin B.
−Removed: in the mononuclear phagocytic system are immune cells that have the capacity to engulf and destroy numerous potentially pathogenic
−Removed: materials and organisms within the body.
−Removed: These cells are found in almost every site of the body, save a few ‘immune privileged’
−Removed: eyes, fetus, and testes).
−Removed: Such cells help with non-specific (innate) immune defenses as well as help initiate specific
−Removed: (adaptive) immune responses.
−Removed: Thus, they play a critical role bridging the gap between innate and adaptive immune responses.
−Removed: Our core capabilities combine the use of lipids as active pharmaceutical ingredients (API) and the use of lipids in “cochleate-shaped”
−Removed: lipid-crystal nano-particle drug delivery vehicles.
−Removed: Therapeutic applications of our proprietary delivery technology were initially
−Removed: focused on the delivery of several potent and highly efficacious anti-fungal and anti-bacterial agents which are currently still
−Removed: associated with serious side effects, including irreversible toxic effects on kidney and hearing function.
−Removed: We believe our technology
−Removed: has the potential for targeted delivery of these agents, which positions us to be at the forefront of dealing with these very
−Removed: serious problems.
−Removed: We have now also expanded our research and development efforts for our LNC platform delivery technology to focus
−Removed: on the delivery of a wide range of therapeutic treatments, in particular those in the oligonucleotide space (siRNA, DNA, antisense
−Removed: DNA, mRNA, and CRISPR-Cas9).
−Removed: We continue to push forward our business development efforts to further expand our collaborations
−Removed: across pharma and biotech companies who have innovative therapies with delivery challenges which may be addressable with our LNC
−Removed: platform delivery technology.
−Removed: A key innovation of our cochleate delivery technology is our ability to package medication inside lipid-crystal
−Removed: particles without leaking.
−Removed: Because of their crystal nature, these particles are truly solid and hold on tightly to their medication
−Removed: This is where the cochleate delivery technology differs markedly from other lipid-based delivery technology, such as
−Removed: liposomal delivery.
−Removed: Liposomes are liquid delivery systems which typically leak some of their drug content into the circulatory
−Removed: system, thus still exposing vulnerable organs and tissues to potential toxic effects.
−Removed: Keeping potentially organ-toxic medications
−Removed: inside the lipid-crystal particles significantly differentiates our cochleate delivery technology from other drug-delivery approaches.
−Removed: The size of our individual cochleate lipid-crystals is typically in the range of 50-500 nm.
−Removed: This is very small
−Removed: and by comparison close to the size of a large virus or a small bacterium.
−Removed: Our body produces several cell-types that are designed
−Removed: to remove viruses and bacteria from our system.
−Removed: These cell types, such as macrophages, are part of our immune system and “swallow”
−Removed: the bacteria and viruses they encounter in order to protect us from infections.
−Removed: Because of the size our lipid-crystal cochleate
−Removed: particles and the phospholipid surface structure (the cell membranes of bacteria are also made up from phospholipids), macrophages
−Removed: tend to absorb these cochleate particles very well.
−Removed: Many drugs that are currently on the market are only effective in treating diseases when administered intravenously.
−Removed: For example, many anti-infective drugs must be administered intravenously in order to be effective.
−Removed: IV administration presents
−Removed: several challenges to care, such as risk of infection, patient discomfort from injections, and higher cost of care than anti-infective
−Removed: drugs that can be taken orally (IV delivery must be performed by a doctor or nurse, often within a very expensive hospital setting).
−Removed: Although several technologies have been used to attempt to convert IV drugs to orally delivered medications, success has been
−Removed: limited due to the difficulty in achieving adequate bioavailability (i.e., the amount of drug that is absorbed into the body)
−Removed: with oral formulation.
−Removed: We believe that the unique cochleate crystal-structure in our platform technology protects the drug from

+Added: total EPA and total DPA were substantially higher for LYPDISO compared with Lovaza.
+Added: Similar findings were noted for free EPA and
+Added: Under fed conditions, the absorption of Lovaza improved substantially, while that of LYPDISO improved slightly.
+Added: omega-3 levels (EPA + DPA + DHA) were approximately 50-60% higher with LYPDISO than with Lovaza after a high fat meal.
+Added: in the fasted state LYPDISO was very well absorbed, with only a minimal food effect, whereas Lovaza was poorly absorbed unless
+Added: taken with a high fate meal, this indicated that the free fatty acid formulation of LYPDISO provided improved absorption with
+Added: a minimal food effect and without the need for a high fat meal for effective absorption.
+Added: Key endpoints and assessments include
+Added: PK parameters (e.g., AUC, C max , T max , t 1/2 ) for total EPA, DHA and DPA and comparison of PK parameters
+Added: for LYPDISO in the fasted and fed (high fat meal) state.
+Added: August of 2020 we held an End-of-Phase 2 Meeting with FDA to review the results of the preclinical and comparative bioavailability
+Added: studies, and to discuss issues related to an approval in SHTG.
+Added: The FDA provided feedback that:
+Added: 1) with these preclinical and comparative
+Added: bioavailability data in-hand, a 505(b)(2) registration pathway was reasonable;
+Added: 2) a single placebo-controlled Phase 3 trial would
+Added: likely be sufficient to establish efficacy in SHTG, and 3) the necessary safety database for approval could be supplement with
+Added: other patients, including patients with SHTG.
+Added: parallel with the preclinical and clinical studies necessary for FDA approval, we have also recently completed the ENHANCE-IT
+Added: study, an additional clinical study intended to differentiate LYPDISO from Vascepa, widely considered the leading prescription
+Added: omega-3 drug.
+Added: ENHANCE-IT was a randomized, open-label, parallel-group, crossover study designed to assess and compare the effects
+Added: of LYPDISO and Vascepa on lipid markers and blood omega-3 levels.
+Added: It included 100 adult men and women with elevated triglycerides
+Added: (150-499 mg/dL);
+Added: approximately 58% of study subjects had TGs ≥ 200 mg/dL.
+Added: The study protocol involved two 28-day treatment
+Added: periods, with a washout period of at least 28 days between treatments and was conducted at 8 sites in the U.S.
+Added: LYPDISO and Vascepa
+Added: were each given as 2g twice daily with food in accordance with the approved Vascepa labeling.
+Added: Baseline and post-treatment measurements
+Added: included triglycerides, Total-, LDL-, VLDL-, HDL- and non-HDL cholesterol, apolipoproteins A1, B and C3 and PCSK9, as well as
+Added: An additional important endpoint was omega-3 blood levels, including EPA, DPA, DHA and total levels for both treatment
+Added: The primary endpoint in ENHANCE-IT was the percent change from baseline to end-of-treatment in plasma triglycerides.
+Added: The first subject as randomized in June of 2020, enrollment was completed by the end of August, last-patient, last-visit was at
+Added: the end of November and the database was locked in mid-January 2021.
+Added: February 2021 we announced topline data from the ENHANCE-IT trial.
+Added: Two analysis populations were prespecified in the statistical
+Added: analysis plan:
+Added: a pharmacodynamic (PD) population (94 patients), and a per-protocol (PP) population (82 patients).
+Added: The PD population
+Added: included all subjects for whom estimation of pharmacodynamic parameters was possible for both treatment periods, regardless of
+Added: study drug compliance.
+Added: The PP population included patients from the PD population, with the added stipulation of at least 80%
+Added: compliance with study medications (verified by pill counts), with no clinically important protocol violations or deviations.
+Added: PP population group was expected to have less variability due to poor study drug compliance, and to provide a more accurate representation
+Added: of how each study drug (LYPDISO or Vascepa) would perform when taken as directed.
+Added: topline results from ENHANCE-IT were as follows:
+Added: primary endpoint (% change in TG from baseline) was numerically greater with LYPDISO vs.
+Added: Vascepa (21.9% vs.
+Added: 15.7%) but did
+Added: not meet statistical significance in the prespecified PD population.
+Added: Statistical significance as achieved in the PP population
+Added: EPA concentrations, absolute change from baseline and % change from baseline were all significantly higher with LYPDISO than
+Added: with Vascepa, in both the PD and PP populations (46% relative percent increase change from baseline EPA with LYPDISO vs.
